To request a proposal, we encourage you to submit an event questionnaire by completing the following simple steps:
  1. Click on the button below to open the questionnaire, which is presented as a PDF fillable form.
  2. Save the blank PDF file on your desktop.
  3. Fill in the information requested on the desktop version.
  4. Resave the PDF form after you have completed it and attach the PDF file to an email message.
  5. Send the email to info@pivotalevents.com. You will receive a receipt confirmation email within 48 hours.


The questionnaire is compatible with Adobe Reader Versions 7.0 or 8.1. If you need to download the free Acrobat Reader Software, you may do so at www.adobe.com/products/acrobat.
